In consideration for my (or my child’s) participation in any activities or instruction provided by Empower Martial Arts, I acknowledge the inherent risks associated with martial arts training and agree to assume full responsibility for any injuries or damages, known or unknown, which may occur as a result.

I affirm that the participant named above is physically capable of participating and does so voluntarily.

I release and hold harmless Empower Martial Arts, including its officers, employees, instructors, agents, and representatives, from any and all claims, demands, or causes of action, whether arising from negligence or otherwise, related to injury, illness, or property loss during participation.

MEDICAL RELEASE

I authorize Empower Martial Arts staff to obtain necessary medical treatment for the participant in the event of an emergency. I understand and agree that I am responsible for all medical expenses incurred as a result of such treatment.

PHOTO / VIDEO PERMISSION

I grant permission for photos or videos taken during Empower Martial Arts activities to be used for promotional or marketing purposes, without compensation or additional consent. I understand these may appear in print, online, or social media formats.

CANCELLATION

Membership cancellation requires submission of a termination form at least seven (7) days before the next scheduled payment.

By submitting the form, you agree to terminate your membership and forfeit your current tuition rate. No prorated refunds will be given for unused time, and cancellations submitted less than 7 days in advance may result in one final payment.

NO REFUND POLICY

All memberships are monthly and non-refundable. Members understand that tuition and fees are due regardless of attendance or withdrawal. No credits or refunds will be issued for missed classes, absences, or early withdrawal from the program.

COVID-19 WAIVER

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk of exposure or infection by participating in Empower Martial Arts programs.

I understand such exposure may result from the actions, omissions, or negligence of myself or others, including staff, students, and families. I agree to assume all risks and release Empower Martial Arts and its representatives from any claims related to COVID-19 illness or its effects, whether occurring before, during, or after participation.
